The HFMS is based loosely on a functional scale developed for boys with Duchenne Muscular Dystrophy (Scott & Hyde 1982) which, as a scale, was not a reliable indicator of change in weaker children. For this reason this particular scale was developed. Activities included in the scale were: rolling, sitting, lifting head from prone and supine, getting up from lying, propping on arms, 4 point kneeling, crawling, standing and stepping. An initial study determined that these items were achieved in normal infants more than 29 months of age and a second phase of the study performed the test 120 times on 58 patients with SMA. This resulted in the test items being ordered in terms of difficulty to gain a measure of grading for SMA.
